MISSION STATEMENT AND PURPOSE


The Northwest Occupational Medicine Center, LLP provides a spectrum of services to evaluate and rehabilitate individuals with a special focus on injured workers and to prevent workplace injuries.

Programs advocate early intervention, consultation with workers, employers, insurers, health-care providers and managed care organizations; worker self-management, rapid return to work and resolution of claims to prevent long-term disability.

MEET OUR STAFF

David Craddock, BS, COHC

David holds a Bachelor of Science degree in Health Promotion/Fitness Management, with a minor in Health/Physical Education from Southern Oregon University. Additional training includes a national certification in occupational hearing conservation (COHC) from CAOHC. David joined NWOMC in 1993 and is currently employed as a Rehabilitation Exercise Specialist. He has specialty training in the areas of spirometry, body composition, audiometric and vision screening. His background also includes over 12 years of extensive work as a personal trainer.

David has competed in both bodybuilding and power lifting and has won awards and titles in both events. He enjoys spending time with his family and friends, and also has personal interests in weight training, collecting unique watches, and commemorative stamps.


Kimberly Goodale, Psy.D.

Dr. Goodale holds Master's and Doctoral degrees in Clinical Psychology from Pacific University. During her doctoral training, she completed practicum training at St. John's Medical Center, conducting assessments and consultations with chronic pain patients. She completed her predoctoral internship at the Veteran's Administration Hudson Valley Healthcare System in New York, where she gained extensive experience working with patients with a wide range of medical and psychiatric issues. Dr. Goodale has also published and presented research on a variety of health-related psychological issues. She is currently a Psychologist Resident at NWOMC.

Dr. Goodale enjoys hiking, running, camping, waterskiing, going to movies, and spending time with friends and family doing all of the above.


Marcel Gosé, O.T.R./L

Marcel received a Bachelor's degree in Occupational Therapy from Pacific University in 1996, and has completed extensive coursework in Exercise and Sport Science at Oregon State University. Marcel's experience in rehabilitation is varied and extensive and includes acute rehabilitation, geropsychiatric care, orthopedics, cardiac care and outpatient and assisted-living care. Additionally, he has served as a Regional Rehabilitation Program Director coordinating services for skilled nursing facilities and, most recently, worked in sub-acute geriatric rehabilitation prior to joining NWOMC in November, 2003. Marcel recently received extensive training to perform physical/functional capacity evaluations through a nationally recognized program. He is also a Certified Personal Trainer and a nutrition specialist.


John Hamby, M.D.

Dr. Hamby completed medical school at Oregon Health Sciences University in 1985. He completed his medical internship at Naval Hospital Oakland, in Oakland, California. After serving 3 years as a general medical officer in the United States Navy, he returned to Oregon where he worked for seven years as the medical director for Willamette Falls Immediate Care and Occupational Health. He subsequently practiced occupational medicine with Columbia Occupational Health Associates, Legacy Occupational Medicine and Cascade Occupational Health Associates. In 2003, he began a practice specializing in pain management, with Progressive Rehabilitation Associates, in Eugene/Springfield and Portland. He has been a member of the NWOMC staff since January, 2005.


Michael Leland, Psy.D, CRC

Dr. Leland holds a Master's degree in rehabilitation counseling and a Doctorate in Clinical Psychology. He is a licensed psychologist in Oregon and Washington since 1996. He has held also national certification as a rehabilitation counselor since 1979. He has extensive professional experience as a rehabilitation counselor and evaluator, rehabilitation educator, and more recently as a medical psychologist. He has had middle and senior management experience in vocational and head injury rehabilitation facilities, as well as a long-term disability insurance company. He has authored numerous professional articles, book chapters and research reports. He has coordinated or presented at over 130 conferences, training programs and workshops nationally. He is currently Co-director of Northwest Occupational Medicine Center, Inc.


Brad Lorber, MD

Dr. Lorber received his undergraduate education at Lewis & Clark College and then went on to complete his MD at Oregon Health Sciences University. He completed his medical internship at St Vincent's Hospital. His residency was in Physical Medicine and Rehabilitation at Long Beach Veteran's Administration and UC Irvine/Long Beach Memorial Hospital. He was employed for several years with Neuromuscular Consultants in Sacramento before returning to Oregon to serve as the Assistant Medical Director and now Medical Director at NW Occupational Medicine Center in December of 1999. He is Board Certified and holds Diplomate status with the American Board of Physical Medicine and Rehabilitation. He is a Fellow with the American Academy of Physical Medicine Rehabilitation. He also has specialty training in Electrodiagnostic medicine including nerve conduction studies. He currently serves as Co-director of Northwest Occupational Medicine Center, Inc.


Luke Patrick, Ph.D

Dr. Patrick holds a Master's degree kinesiology, with training emphasis in health behavior and the psychology of exercise and sport. He received his Ph.D in counseling psychology at the University of Iowa, where he focused his research and clinical training on medical psychology. He completed his predoctoral internship at the Portland VA Medical Center in Portland, Oregon. He has published and presented original research in the psychology of athletic injury, exercise adherence, selection of medical professionals, and long-term outcomes for multidisciplinary pain treatment. He is currently the Director of Psychology Services at Northwest Occupational Medicine Center.

Dr. Patrick's extracurricular interests include enjoying Oregon's many wonderful outdoor opportunities and spending time with his wife, two great kids, and Bailey the Labrador non-Retriever.


Allisyn Smith, DPT

Allisyn received her Bachelor's degree from Linfield College in 1997, Master's degree in Physical Therapy from Western University of Health Sciences in 2001, and Doctorate in Physical Therapy through Boston University in 2006.. She has experience in acute and subacute orthopedics as well as cardiac, cancer, pulmonary, and neuro rehabilitation. Additionally, she has a strong background in sports rehabilitation and injury management, having worked with many high-level athletes and celebrities as a graduate student. Allisyn is a Certified Pilates Instructor (CPI), and has received extensive training to perform physical/functional capacity evaluations through a nationally recognized program. She has been a member of the NWOMC staff since March of 2003 and is the current program director for the chronic pain management, work hardening, and work conditioning programs. She is a licensed physical therapist in Oregon and California.

Allisyn and her husband enjoy travel, cooking, running, hiking, and spending time with family, pets, and friends.
